Your Medicare number
Your current address and phone number
A valid email address
Documentation verifying your GHP coverage through your or your spouse's current employment.
Once the member is done filling out the application, they will get a notification in the mail of their application status. They can even call Medicare for their status at 1-800-633-4227.
